Required initial information
Enter the patient’s age and sex to get started.

Note: If you are starting a new patient and still have patient data from the previous one, you can use the Clear Patient button (which will be active).
Care setting
The default care setting is Primary.
Select care setting: primary, secondary, or tertiary. Focus on common conditions with Primary (the default). Ignore the incidence of the disease by selecting Tertiary. Use Tertiary when a patient has already had many conditions ruled out, or where the disease may be newly described, and the incidence is not well known.
Optional context
Define emergence. onset over minutes is very different from onset over years.
Add optional family history and exposure. The transmission menu below allows a quick and easy way to indicate transmission, as needed.


Starting a patient from saved information
If you or a colleague previously saved the patient’s information you have several options to get back to where you were.
- Output/Patient Summary PDF
- Use the Load option on the top black navigation bar to load the file.
- Click the “Click to re-open the patient”.
- Output/Patient link (computable summary) or the link in the Output/SOAP note
- Use the Load option on the top black navigation bar to load the link.
- Click the link.
